Supported Living Open Framework

This document sets out a service specification relating to the provision of care and support for vulnerable adults living in the East Riding of Yorkshire who have been assessed by the Community Wellbeing Team as being in need of care and support. The Supported Living Open Framework is to meet the identified needs of those vulnerable adults whose need for care and support normally stems from them having a identified learning disability and or neurological condition.

The Agency must provide high quality outcome-focused and person-centred care, comply with all regulations, and work collaboratively with the Council and other stakeholders.
